Pathways, in conjunction with Duncan Regional Hospital,

is requesting your assistance in conducting a community health needs assessment.  Under new health care legislation, Pathways, Duncan Regional Hospital and Jefferson County Hospital are required to conduct a community health needs assessment every three years.   Your input is vital for the success of the process.

We have partnered with the County Health Department to facilitate the assessment.  The process will consist of three phases with the first being completing the community health needs assessment (CHNA).  The second will be collecting and evaluating the data from the CHNA and lastly, working with a committee to develop the community health improvement plan (CHIP) for 2019-2021.

Again, your input is extremely important and we would greatly appreciate your participation in this important initiative.  Thank you for your valuable service to our counties.

Please email Kimbra.whaley@duncanregional.com or call 580-251-8558 with any questions.

March School Board Agenda
The Central High 4-year old program will have their pre-enrollment on Thursday, April 11th from 4:00 p.m. – 6:00 p.m. in the auditorium.

All students entering the 4-year old program must be 4 years old on or before Sept. 1, 2019.  All students need to have had the following immunizations: 4 DTap, 3 Polio, 1 MMR, 3 Hepatitis B, 2 Hepatitis A, and 1 Varicella.

Please bring your student with you to enrollment. You will need to bring a copy of your child’s birth certificate, immunization records, social security card, proof of residency in the Central High school district (a utility bill, rental agreement, etc.), and an affidavit of custody or other legal documentation if the child is in the custody of non-parental guardians (this ensures protection of the student) when you enroll.
